File a Complaint

In Arizona, if a patient or pharmacy feels that an insurance company or their pharmacy benefit manager has violated state law, they can file a complaint with the Arizona Department of Insurance.

You Will Need:

  • Insurance Information

  • Any applicable documents or records of the violation (claims, emails, letters, etc.)

  • Any other data or information that helps prove the violation

The more legitimate, fact-based complaints the state's DOI receives, the more likely it is that they will officially Investigate and Enforce state law.

Help Put A Stop To PBM Abuse of Arizona Law

PBM and their insurance parent companies need to be held accountable for actions that hurt our local pharmacies, employers, and patients. If you know a PBM or its health insurance counterpart has violated Arizona state law, file a complaint with the DOI now.
